Police & ForensicsCrash diagram, location and damage codes

You put the crash on paper: where exactly it happened, which way each vehicle was going, where each one was hit and the diagram of the crash, on the New York police crash report, form MV-104P. It follows the DMV instructions for the damage codes, the crash diagram and the crash location of the report.

Scenarios

  • A single-vehicle crash.
  • A vehicle striking a pedestrian or a bicyclist.
  • Two vehicles colliding at an intersection.
  • A crash between two intersections on a state highway.

The steps

  1. For each unit, enter in Box 1 the code (1 to 16) of the first point of impact on the unit diagram, for example 9 for the tail light area on the driver's side.
  2. Enter in Box 2 the area of the unit with the most damage, and use Boxes 3 to 5 for more damage codes if needed; for a motorcycle, truck or bus, the unit diagram stands for that vehicle.
  3. If the damage is "Other", enter code 19 and explain it in the officer's notes.
  4. With one vehicle, a vehicle striking a pedestrian, bicyclist or other conveyance, or three or more vehicles, enter 9 and draw the diagram in Box 9, labeling everything relevant to the crash.
  5. With two vehicles, enter one diagram code from 0 to 8 or draw the diagram in Box 9.
  6. Determine from the investigation the direction each unit was traveling and enter it in Boxes 23 and 24; for a parked vehicle, the direction it was facing.
  7. Enter in Boxes 25 and 26 what each unit was doing just before the crash.
  8. Enter the first event of the crash in Box 28 and each unit's second event in Boxes 29 and 30.
  9. On state and interstate highways, copy the numbers of the nearest reference marker exactly, one per box, without transposing them.
  10. Enter the crash coordinates in decimal degrees, latitude before longitude.
  11. Enter the county, checking city, town or village, and the route number or street name where the crash occurred.
  12. Inside the crosswalk box of an intersection, enter the intersecting street; between intersections, enter the distance in feet or miles and the direction from the nearest intersecting street.
  13. If the crash was reconstructed, check the box and print the reconstruction officer's name in the officer's notes.

To pass

  • Every crash is located precisely: the state uses the locations to find high crash locations and target highway improvements.
  • The diagram labels what matters to the crash, and every code marked with an asterisk is explained in the officer's notes.
