20 cards
NY damage codes, Box 1?
The first point of impact on each unit, as a code from 1 to 16 on the unit diagram.
Damage code 9 on the unit diagram?
The tail light area on the driver's side.
Damage codes, Box 2?
The area of the unit with the most damage.
Damage that fits no area code?
Enter code 19, Other, and explain it in the Crash Description/Officer's Notes.
What are Boxes 3, 4 and 5 for?
Extra damage codes, if needed.
The unit is a truck, bus or motorcycle: which diagram?
The same unit diagram, read as if it showed that type of vehicle.
When do you check the Cost of Repairs box?
When repairs to any one unit or property will cost more than $1,000.
Which two towing boxes does the NY report have?
Vehicle Towed, and Vehicle Towed Due to Disabling Damage.
How does MMUCC mark the initial contact point?
With clock points 01 to 12, plus top or undercarriage; contact to the grille is clock point 12.
MMUCC Extent of Damage is based on what?
How the damage affects the vehicle's operability, not the cost to repair it.
Disabling damage?
Damage that keeps the vehicle from leaving the scene normally after simple repairs, so it is towed, carried or assisted away.
Functional damage versus minor damage?
Functional affects how the vehicle or its parts work without disabling it; minor does not affect operation.
A tow truck pulls a car out of a ditch and it drives away: towed?
No; towing help without removing the vehicle from the scene does not count as towed.
When do you send an Amended Report?
Whenever a Police Crash Report already sent to the DMV needs a change.
Two classic reasons to amend?
A hit-and-run driver is identified later, or an injured person dies within 1 year of the crash.
How must the amended report's identifying data look?
Exactly like the original report, even if the original had an error.
What do you fill in on the amended report?
The Amended box, the original date and time, all original driver and registration data, and the original location and county.
Where do the actual changes go?
Only in the Crash Description/Officer's Notes; a new crash date, for example, goes there while the top keeps the original.
Amending an old MV-104A or MV-104AN report?
Use the new MV-104P with the change noted in the notes; only the amended report is sent.
Amending an MV-104D fatal crash supplement?
Write the change in its Additional Information and in the MV-104P notes, and send the MV-104P with it.
