Police & ForensicsLicense, registration and insurance on the crash report

You take each driver's license, the vehicle registration and the insurance card and copy them onto the New York police crash report, form MV-104P, exactly as printed, so the state can match the crash with the right license and vehicle records. It follows the DMV instructions for the driver, registration and insurance boxes of the report.

Free to start

Scenarios

  • Two vehicles with New York licenses, registrations and insurance cards.
  • A driver with an out-of-state insurance company.
  • An unregistered vehicle.
  • A police or fire vehicle involved in the crash.

The steps

  1. Fill in the driver section when a running vehicle has property damage over 1,000 dollars, someone is injured or killed, or an e-bike or e-scooter is involved.
  2. Print each driver's information exactly as it appears on the license document.
  3. Enter the license or non-driver ID number and the state, province or country that issued it.
  4. Check the license class and permit class that match the document, and mark it if the driver was driving outside that class.
  5. Mark the driver's license status at the time of the crash.
  6. Enter the name exactly as on the license, the date of birth, the sex (M, F or X) and the address.
  7. Print the registrant's information exactly as it appears on the registration document: Unit 1 is always the vehicle of Driver 1.
  8. Enter the plate number and the state of registration; for a police, fire or other exempt public vehicle, write POLICE, FIRE or the right description instead of the plate.
  9. If the unit is unregistered, enter its vehicle identification number (VIN).
  10. Enter the vehicle year, make and model and the VIN.
  11. Enter the insurance company name and the policy number.
  12. Enter the 3-digit code from the New York or New Jersey insurance identification card; with no code, such as an out-of-state insurer, write the company name in the officer's notes and leave the code box blank.
  13. Describe in the officer's notes any name or address that differs from the license or registration, such as a married name.

To pass

  • The driver data matches the license document exactly, because it is used to link the report to the right license record or to create a new one.
  • With three or more units, additional report forms are used, numbering the units 3, 4 and so on.
