How to reassess pain after a PRN medication
Other platforms explain it with text and video. In Making Synapses you practice it on the patient.
An as-needed (PRN) dose is only finished when you know whether it worked. Reassessment tells you if the patient needs a different plan or is having side effects.
Step by step
Record a baseline
Before the dose, rate the pain with a scale the patient understands and note where it is and what it feels like. [1]
Agree on a goal
Ask what level of pain lets the patient rest, walk or do therapy. Function matters as much as the number. [1]
Time the recheck
Reassess at the drug's expected peak, usually 15 to 30 minutes after IV and 30 to 60 minutes after oral doses, or as your policy states. [2]
Use the same scale
Compare like with like. If the patient cannot self-report, use the same behavior-based scale as before. [1]
Check side effects
Look at sedation, breathing, nausea, itching and dizziness, especially after opioids. [2]
Act on the result
If pain is still above the goal, try comfort measures and call the prescriber for a plan change. Do not wait until the next dose is due. [2]
Document
Record the reassessment time, score, side effects and actions taken. [1]
Common errors
- Never going back to recheck the pain.
- Switching to a different pain scale for the recheck.
- Writing pain relieved without asking the patient.
Typical reassessment timing (follow your facility policy) [2]
| Route | Recheck at about | Why |
|---|---|---|
| IV | 15 to 30 minutes | Fast onset and early peak |
| IM or subcutaneous | 30 to 60 minutes | Absorbed from muscle or fat |
| Oral | 30 to 60 minutes | Needs time to be absorbed from the gut |
Practice it in Making Synapses
In Making Synapses you do this on the patient, not on paper: every step goes on the checklist with a score, and in Teach mode Blocky tells you what comes next.
